You have to download/install the game to your computer first, and you launch the game from there. Look for the link to download.
Once you've launched, you create a user name/password. When you're logged in, you'll start off in the lobby. Look to the list of the rooms on the right (may have to click on a plus sign to open a folder), select a room, and click Join.
The adult rooms here tend to be R or X rated, in a spirit of fun. The non-adults tend to be "clean" or innuendo.
